The final report by the Commissioner of Railway Safety (CRS) found that the accident was due to an error in allowing the goods train to proceed on a section despite the presence of the Kanchanjunga Express. This error was compounded by a faulty memo that allowed the goods train to pass all signals without any cautionary orders. The goods train was traveling at 78 km/h when it detected the Kanchanjunga Express, prompting the loco pilot to apply emergency brakes. Despite this, the train could only decelerate to 40 km/h before the collision. The report highlighted Anil Kumar's efforts to adjust the throttle 10 times within 5 minutes, demonstrating his alertness.

ki jaanch ki aur pata chala ki Anil Kumar ki koi galti nahi thi. Unki patni Roshni ne kaha ki unko accident ke kuchh hi ghanto mein doshi thahra diya gaya tha, jisse unko bahut dukh hua. CRS report me kaha gaya hai ki goods train ko section par jaane ki ijaazat galti se di gayi thi, jisse yeh accident hua. Goods train 78 km/h ki speed se chal rahi thi aur jab usne Kanchanjunga Express ko dekha toh brake lagae. Train 40 km/h ki speed tak aayi par phir bhi accident ho gaya. Report me kaha gaya hai ki Anil Kumar ne 5 minute me 10 baar throttle adjust kiya tha, jisse unki alertness ka pata chal raha hai.
