Didn't see the video, because I've read about the CRS report. And a few things are concerning.

Enough walkie-talkies weren't available with the division. Isn't that the most basic tool of communication between the people responsible for operations.

If

the automatic signal wasn't working since 5:30 in the morning, why no steps were taken to rectify it. Or why the section was converted temporarily to absolute block.

Even if TA/912 was issued, how 5 trains were allowed in the block section at the same time. And even in that the MPS wasn't mentioned.

Were the staff untrained about protocols, because except Kanchenjunga no train followed the prescribed speed limit or waiting for a few minutes on every signal. Shouldn't a loco pilot know these things which even a lot of railfans know.

Why the goods loco pilot was overspeeding, was he under some time bound orders from above, because it was a clear day and a slow train could've been stopped in time after spotting another train standing ahead.

The same thing happened with the Gonda derailment, everyone knew that the tracks were unfit, but no steps were taken, even caution order wasn't issued. Now everyone is busy shifting blame on each other.

Till when everyone from the top brass to the grassroot workers will show incompetence. And till when people's life will be treated cheaper than a PR stunt. Someone needs to take responsibility and take it as soon as they can.

The video details exactly the same.
Systemic failures at multiple levels caused the accident.

Looks like not following speed restriction in TA912 was pretty common there as neither the document given mentioned speed restriction to follow neither were the LPs properly trained in that matter so whoever knew it followed and others just bypassed.

Relying on mobile networks for communication in such critical work environments is a horrible idea. The zone involved should be severely punished for not providing walkie talkie to the LPs and guard.

Multiple accounts of automatic signalling failures being reported and no proper action taken to find out why it's failing so often and fixing it.

Kavach system should be implemented across Railway network on priority basis.

People travel with peace of mind and safety thinking that Indian Railways is a robust organisation with utmost priority to safety operated by professionals but these things coming out of the report are pretty frightening. Feels like many things are like "jaise chalta hai vaise chalne de" in Indian Railways.
If such things continue people will loose faith in Railways and look for alternative options.

Agree Agree:true words. Agree
If everything just works in the 'chalta hai' sense, then what are those training sessions for. Anyone can learn how to operate something, how to operate it in the right way is the sole purpose of training.

I can't rely on mobile networks when I'm traveling on a train and they're relying on it for operating the same train. This is just another disaster waiting to happen.

Automatic signaling, Kavach, everything is computer based. And no matter what RM says about it being fool-proof for media clout, computers and machines can and will fail. Just look at how a faulty update in the Windows security system brought half the world to a standstill.

You can't rely on computers or technology completely and call it a day. I agree that airline pilots depend a lot on technology, but they're trained on how to control things manually in case everything goes south. And it's not like they were inherently prepared.

Accidents happened, the air travel authorities around the world took note and brought up changes. Operating people were trained appropriately according to the lesson learnt. Why can't railways do the same?

They just love to shift blames on the person inferior to them, even CRS reports subtly do the same. Even we forget everything and the system remains the same, waiting for another disaster to strike.

I'll like to disagree with you here. Not because this is a politically not-correct term, but because the root of the problem lies somewhere else.

People don't want to work nowadays, everything has come down to making a work look like it has been done. Everyone just likes to check the list and get back to their phones. And here also lies the biggest downside of privatization which many people miss deliberately.
...

Everyone attributes the privatization to something like in IT industry. But in small scale jobs, it's just long hours and minimal pays, with no job security. And you won't ever give your 100% to something like that. Tell me why a gangman will toil whole day if he's outsourced, because now he has no liability to the organization.

And the elephant in the room, everyone says government employees don't work. That's the reason I say ministers and top bureaucrat should be held responsible. If the top brass will be held responsible, they'll keep their subordinates in check. And services will be better automatically.

But when every question raised by someone is quashed by fellow citizens in the name of petty politics, we can't expect any change anytime soon.
